We are looking to partner with you to service communities in need and provide quality patient care while improving the patient experience. Our telehealth and home care services can:
Help eliminate hospital readmissions by timely follow up with high risk and recently discharged patients.
Offer personalized care to seniors and disabled persons by partnering with independent living and assisted living facilities, skilled nursing facilities, hospice, home health agencies and group homes with the goal to reduce unnecessary emergency room visits.
Serve as extenders for primary care and specialty providers needing additional coverage after hours or during periods of capacity limitation. We refer all patients back to their PCP or referring doctor to maintain continuity of care and promote integrated care.
Provide on-site medical care at organizations and for events to include: first aid stations, testing and immunization clinics, physical examination clinics and more.
